Dementia Link Worker

This job description is a statement of duties for a Dementia Link Worker/Support Assistant, it is not an exhaustive list of duties that will be required. Bramble Home Care may revise this job description from time to time to meet the needs of the business including anything that the management deem to be reasonable.

 

Accountable to: Branch Manager

 

Job Purpose:

 

· To provide guidance and support to the Care Support Staff with managing complex Dementia Care.

· To provide and demonstrate care in accordance with current best practices, according to policy and procedures, agreed standards, legislative requirements, relevant regulations under the direction of the Manager.

· To maintain skills at a current level and undertake such training and development as may from time-to-time be required to maintain practices as up to date.

· Report promptly any concerns/complaints or grievances to their Line Manager/Registered Manager

· To have an active awareness of personal responsibility and accountability for helping to ensure that a safe working environment is maintained in accordance with legislation.

· To work alongside the Dementia Lead and other Dementia Link Workers.

· Represent Bramble Home Care and promote its brand.

 

Main Duties and Responsibilities:

 

· To be punctual, smart at all times whilst representing Bramble Home Care.

· Providing hands on care and advice to our complex Dementia Clients.

· Providing personal care to our clients including washing, dressing, assisting and dispensing medication and meal preparation.

· To provide care to Service Users in accordance with Policy and procedure.

· To give on-going support to new and existing staff around complexities of Dementia ensuring the client remains at the heart of what we do.

· To carry out duties to enable Bramble Home Care to continue to comply with and meet all standards relevant to national regulation and local authority requirements.

· To have an active awareness of personal responsibility and accountability for helping to ensure that a safe working environment is maintained in accordance with legislation.

· To report to the Line Manager the progress of individuals, incidents that have occurred and general observations daily where necessary and at Team meetings.

· To maintain confidentiality throughout the service as stated in the Carer and Support Worker’s Information Guide.

· Attend mandatory training updates with One Gloucestershire to keep up-to-date and ensure best practice is always used within the Dementia Service.

· Attend Staff Meetings and Mandatory training as and when required.

· Any other duties applicable to the post, as requested by management.

 

Skills and Qualifications:

 

· Dementia Link Worker Award

· NVQ Level 2 or 3

· Supervisory experience desirable

· Good record keeping skills.

· Ability to work under pressure and unsupervised in a fast-paced community environment.

· Exceptional communication skills
